Book an appointment

Booking Instructions

How to Book Your Assessment


We aim to make the booking process as simple and supportive as possible. Please follow the steps below to get started:


1. Complete a History Form


To begin, we ask you to complete a background form so we can understand your needs and determine whether an autism and/or ADHD assessment is appropriate. Please choose the correct form below:


  • Complete a Child History Form
  • Complete an Adult History Form

Child History Form


These forms ask for key information about early development, education, behaviour, sensory experiences, and any previous diagnoses or support. You do not need to write in full sentences—bullet points are fine. Our history forms now cover both autism and ADHD as standard.  Please complete both sections. This will enable us to understand your profile and any co-occuring neurodivergence. Please provide as much information as possible.


2. We Review Your Form


Once we receive your completed form, our team will review the information to ensure that an assessment is suitable and recommend the best pathway. If we agree that autism or ADHD identification is the right pathway for you, we will send you an email with a secure booking link.


3. Book Your Collaborative Discussion Appointment


The email you receive will contain a link to our online booking system, where you can select a date and time for your collaborative discussion appointment. This is a key part of the assessment and involves a detailed conversation with one of our clinicians to explore your experiences, strengths, and challenges. The session typically lasts 60 to 90 minutes.


4. We Will Schedule Your Developmental History/Informant Appointment


After you have booked your collaborative discussion, our team will match you with a second clinician to complete the developmental history/Informant component. We will contact you separately to confirm the date and time for this session. This approach allows us to ensure a good fit with your clinician and avoids double-booking.


5. Confirmation and Reminders


Once both appointments are scheduled, you will receive confirmation emails with full details, including times, video links (if virtual), and any preparation needed. You will also receive reminder emails before each appointment.
